INFORMED CONSENT FORM
DISSERTATION STUDY: An Analysis of the Preferred Training Methodology for Adult Learners in Teaching Business
Ethics.
1. I understand that this study is of a research nature. It may offer no direct benefit
to me.
2. Participation in this study is voluntary. I may refuse to enter it or may withdraw
from it at any time without creating any harmful consequences to myself. I understand also that the investigator may drop
me at any time from the study.
3. The purpose of doing this study is to study the effectiveness of different training
methodologies when training adult learners in the topic of ethics.
4. As a participant in this study I will be asked to take part in completing an online
web survey that discusses the type of training that I received (web-based or live presentation) and my preferred style of
learning.
5. Participation in the study will take approximately 30 minutes of my time and will
take place at my office within Wisconsin Physicians Service.
6. There are no human risks associated with these surveys or from the use of the
data for this research project. The research instrument will not have any names
recorded on it.
7. The possible benefits of the procedure might be future training programs for adult
learners will be better tailor to their learning preferences thereby enhancing the experience and results for all concerned
parties.
8. The information about this study was discussed with me by Susan Porter, Compliance
